1. Name

2. Contact Details

3. Availability (i.e. how much notice you might need to be contacted for an event, what time of year you are unavailable etc.)

4. Practical/Special skills, eg. driver’s licence, good at organising groups, strong admin skills etc.

We will then add you to our database of Hospice Fund Volunteers and contact you as events arise.
